Programme Three – Enhanced primary and community care
Through programme three we are working on the following core projects:
- Development of an integrated frailty model to cover both community and acute hospital services as well as looking at our strategy and approach to prevent falls
- Podiatry – reviewing the current model of podiatry to support and improve patients’ quality of life
- Respiratory – improving health outcomes and optimise the length and quality of life for people with and at risk of respiratory disease (breathing problems) including care at end of life
- Development of an integrated neighbourhood operating model by promoting a model of integration that is truly preventative, proactive and person-centred
- Enhanced care in care homes to provide joined up health and care to residents of care homes via a range of in-reach services
- Care packages – improving patient experience and performance of NHS Continuing Healthcare (CHC) assessments and the decision making process
- Acquired brain injury rehabilitation service – developing a cost effective specialist community rehabilitation service for people with acquired brain injury
- Transforming community equipment services by ensuring the delivery of sustainable care equipment services including wheelchairs.
