Programme Four – Intermediate and urgent care

Through programme four we are working on the following core projects:

• Focussing on ‘high intensity users’ to support people to reduce their over dependence on unscheduled care and reduce emergency admissions to hospital
• Recovery at home service – reviewing our current service to maximise independence and improve quality, integration, and sustainability
• Urgent care strategy – improving access to urgent care that meets the needs of local people in the right place at the right time.
