Mistaken medication and inappropriate prescriptions that can do more harm than good are being dealt with, thanks to All Together Better Sunderland.
Where does pharmacy fit?
A team of three pharmacists are now part of a newly introduced Recovery at Home service – one of three strands of All Together Better and is taking referrals from colleagues including social care workers and community matrons, to speak with patients and carers by telephone or in their own home.
The clinical pharmacists – who work from Sunderland Royal Hospital, the Recovery at Home hub at Leechmere and out in the community – carry out a range of tasks including advising people on how best to take their medication, completing medicine reviews and also prescribing to people who may have different symptoms or who have been diagnosed with a new condition and need new treatments.
